Internship in India After MBBS Abroad — NMC Rules 2026 Explained Clearly

internship India after MBBS abroad NMC 2026

One of the most commonly misunderstood aspects of MBBS abroad is what happens after the student returns to India. Most families focus entirely on admission, fees, and FMGE — and underestimate the importance of understanding the internship requirement before choosing a university.

This guide explains exactly what NMC requires for internship after MBBS abroad in 2026 — the foreign internship, the India internship (CRMI), the changed hospital rules, and the complete pathway from returning to India to getting a permanent medical license.

Two Internships — What Most Families Do Not Know

Indian students who complete MBBS abroad face two separate internship requirements, not one. Most families only learn about the Indian internship (CRMI) — the 12-month rotating internship done in India after clearing FMGE. But there is a second one that happens before this — the internship done abroad at the same university where the student completed MBBS.

Internship Abroad — Year 6 at the Foreign University

NMC’s FMGL Regulations 2021 require a minimum 54 months of academic MBBS education plus a 12-month internship — both at the same foreign institution. This is why the program is 6 years total, not 5.

The foreign internship must be completed at the same institution where the student studied Years 1 through 5. Students cannot do their foreign internship at a different hospital or institution — doing so invalidates NMC eligibility.

For students at KazNMU in Almaty, the internship rotations happen at KazNMU’s own teaching hospitals directly attached to the university. For students at Al-Farabi Kazakh National University, the internship happens at affiliated teaching hospitals. Both are NMC-compliant because the university formally oversees the internship as part of the degree.

Internship in India — CRMI (12 Months After FMGE)

After returning to India and clearing FMGE, the student must complete a Compulsory Rotating Medical Internship (CRMI) of 12 months at a recognized teaching hospital in India. Only after completing both the FMGE and the Indian CRMI does the student become eligible for permanent medical registration.

The May 2026 Hospital Rule Change — Critical for Current Returnees

This is the regulatory change that has caught many recent returnees off guard.

Previously, NMC had issued a one-time permission allowing foreign medical graduates to complete their Indian CRMI at non-teaching hospitals — hospitals that are not attached to a medical college. This was a relief measure introduced during the pandemic and extended multiple times.

NMC let foreign graduates use the erstwhile MCI list of non-teaching hospitals as a one-time measure, extended to May 2026, and when we searched NMC’s notices on 18 October 2026 we found no extension beyond that.

This means students returning to India after FMGE clearance from October 2026 onward must complete their CRMI at recognized teaching hospitals only — hospitals attached to NMC-recognized medical colleges. Non-teaching hospitals are no longer an option for CRMI.

For Rajasthan students, this means the Indian internship must be done at hospitals attached to government or recognized private medical colleges in Rajasthan or any other state. Indasia Global’s Jaipur office can advise on recognized teaching hospitals for CRMI upon your return.

Complete Step-by-Step Pathway — MBBS Abroad to Practice in India

Here is the verified, complete pathway in the correct sequence:

Step 1 — Complete MBBS Abroad (6 Years) Complete 5 academic years plus 1-year internship at the same NMC-approved foreign university. Receive the MBBS degree or equivalent. Collect all academic transcripts, degree certificate, and internship completion certificate.

Step 2 — Apply for FMGE Register for the next available FMGE session at natboard.edu.in. Currently three sessions per year — June, October, and a third session. The FMGE full form is Foreign Medical Graduate Examination, and the exam is now moving to up to three sessions a year, with an additional 31 October 2026 session announced. Appear for and clear FMGE with minimum 150 out of 300 marks.

Step 3 — Collect FMGE Pass Certificate The Pass Certificate opens the Indian part of your training. Under Regulation 4 of the NMC’s Foreign Medical Graduate Licentiate Regulations 2021, a foreign medical graduate gets permanent registration only after a supervised internship in India of at least twelve months. Download and preserve this certificate carefully — it is required for all subsequent steps.

Step 4 — Apply for Provisional Registration Submit application to NMC with FMGE Pass Certificate, foreign degree, internship completion certificate, NEET scorecard, NMC Eligibility Certificate, and passport. NMC issues provisional registration that allows the student to begin CRMI.

Step 5 — Complete 12-Month CRMI in India Join a recognized teaching hospital for the 12-month Compulsory Rotating Medical Internship. The CRMI must cover rotations across Medicine, Surgery, Paediatrics, Obstetrics and Gynaecology, and other major specialties. Must be at a teaching hospital after May 2026 — non-teaching hospitals are no longer permitted.

Step 6 — Apply for Permanent Registration After completing CRMI, apply to the State Medical Council of your home state (e.g., Rajasthan Medical Council for Jaipur students) with CRMI completion certificate and provisional registration documents. Permanent registration is issued and you are eligible to practice medicine in India.

Timeline — How Long Does the Complete Process Take?

From completing MBBS abroad to becoming a licensed practicing doctor in India typically takes 18 to 30 months, depending on how quickly FMGE is cleared.

Best case (clear FMGE on first attempt): Return from abroad → Wait for next FMGE session (1-4 months) → Clear FMGE → Provisional registration (2-4 weeks) → 12-month CRMI → Permanent registration (2-4 weeks) → Start practicing. Total: approximately 15 to 18 months.

Average case (clear FMGE on second attempt): Add 4 to 6 months for the second attempt. Total: approximately 20 to 24 months.

This timeline is comparable to the domestic MBBS internship and registration process, which also takes 12 to 18 months after the degree is complete.

FMGE vs NExT — Which Applies to Students Currently Studying Abroad?

This is the most frequently asked question from families with children currently in Years 1 to 4 of MBBS abroad.

As of October 2026, FMGE is still the licensing exam for foreign medical graduates. NMC deferred NExT on 13 July 2023 until further directions from the Health Ministry, and its What’s New list shows no NExT notice after 8 February 2024. Treat any specific start year you read as a guess rather than a notified date.

For students enrolling in MBBS abroad in 2026 who will return in 2032, the exam they will face depends on whether NExT is implemented by then. Based on current status, this is uncertain. What is certain is that NExT — if implemented — would be a similar licensing exam to FMGE, but with an additional clinical skills component. Strong clinical training at the foreign university is the best preparation for either exam.

How University Selection Affects FMGE and CRMI Success

The connection between university choice and FMGE outcome is direct. Universities with strong clinical training from Year 3 onward produce graduates who are significantly better prepared for FMGE’s clinical question weightage.

Al-Farabi Kazakh National University’s 51.08 percent FMGE pass rate in 2024 and Georgian American University’s 80.33 percent pass rate are not random — they reflect curricula and clinical training programs designed to produce graduates who can pass licensing exams.

The CRMI hospital selection in India also matters. A CRMI at a busy government teaching hospital with genuine patient exposure and rotation supervision produces a more competent and confident doctor than a nominal CRMI at a quieter institution. Choose the CRMI hospital for quality of exposure, not just convenience of location.

Frequently Asked Questions

1. Do I need to do internship in India after MBBS abroad? Yes. After clearing FMGE and receiving provisional NMC registration, a mandatory 12-month Compulsory Rotating Medical Internship (CRMI) in India is required before permanent registration and the right to practice independently.

2. Can I do my Indian internship at any hospital after FMGE? No. Since May 2026, only recognized teaching hospitals (attached to NMC-recognized medical colleges) are eligible for CRMI. The previous one-time permission for non-teaching hospitals has not been extended beyond May 2026.

3. Is the internship done abroad at the foreign university counted toward the Indian CRMI requirement? No. The foreign internship is a degree requirement for the MBBS abroad. The Indian CRMI is a separate, additional 12-month requirement imposed by NMC after FMGE clearance. Both must be completed independently.

4. How many times can I attempt FMGE if I fail? There is no limit on FMGE attempts. With three sessions per year, students can attempt up to three times annually without any restriction on total attempts.

5. What happens if I do not clear FMGE? You can reattempt FMGE in the next session. There is no career deadline — FMGE can be attempted indefinitely until passed. Focused preparation between attempts, particularly on high-yield clinical subjects, significantly improves outcomes on subsequent attempts.

6. How does Indasia Global support students returning from Kazakhstan for FMGE and CRMI? Indasia Global’s Jaipur office provides guidance on FMGE preparation resources, NMC registration process, and recognized teaching hospitals in Rajasthan for CRMI. Our students from KazNMU and Al-Farabi, who have above-average FMGE outcomes, typically require less post-return support than students from lower-performing universities.
